The Boys Season 3 finale spoilers: Is Butcher dead?
When Starlight found out that temporary V kills people, she warned Butcher but he ignored the warnings. Did he die in The Boys Season 3 finale?

We all knew there was going to be some sort of side effect of taking the temporary V. Starlight learned that after three to five doses, people die. Butcher didn’t care about himself, but he did prevent Hughie from taking any more as he realized that Hughie was much like his little brother—who we learned killed himself due to the abuse of their father.
Butcher wouldn’t stop taking the temporary V, though. He wanted to take out Homelander, and he was willing to put his life on the line for that. In the fight at Vought Tower, he ended up fighting against Soldier Boy when he realized Ryan was there.
What happened to Butcher in The Boys Season 3 finale?
By the end of the episode, everything changed. Maeve is no longer a Supe, Black Noir is dead, and Soldier Boy is back in a Sleeping Beauty state. Ryan has chosen Homelander, and they’ve both decided to stand with the Nazis who wanted Stormfront back.
What about Butcher? He is still alive, but it may not be for long. A doctor shared that he only has a year to 18 months left to live. It’s all thanks to the temporary V that has done permanent damage to his brain. There is nothing to save him.
At least, that’s the case for now. Considering Vought is constantly finding something new to create, there is a chance we’ll see some sort of cure. However, it’s also possible that The Boys Season 4 is going to be the end of the road for Butcher.
He is still with the group, but he’s not the de facto leader. The group has become a democracy with Starlight a permanent member with no costume anymore.
